1. Request addressed to the Director of the Pharmacy and Drug Control Department specifying the name, quantity, concentration, and batch number of the medication to be exported.

2. Copy of the prescription or medical report issued by a healthcare facility approved by the Ministry of Public Health in Qatar.

3. Copy of the patient’s Qatari ID card.

The medication must match what is stated in the prescription and medical report.
