1. A certified medical report issued by one of the officially accredited medical institutions in Qatar (Hamad Medical Corporation, Al Rumailah Hospital, Al Maha Specialized Pediatric Care Center, Sidra Medicine).

2. A recent passport-size photograph of the student

1.The student must be a Qatari national with a disability who wishes to study in private schools.

2.The student must fall under one of the following disability categories:

-Intellectual disabilities and their associated syndromes

-Dual disabilities (hearing and visual impairments)

-Multiple disabilities (more than one disability)

-Physical and motor disabilities

-Autism Spectrum Disorder

-Hearing impairment

-Visual impairment

3.The student must have a certified medical report issued by one of the officially approved medical institutions in the state, such as:

-Hamad Medical Corporation

-Rumailah Hospital

-Al Maha Center for Children with Specialized Care

-Sidra Medicine

3.The report must not be older than two years from the date of submitting this request.
