At the time of application:

1. Proposed trade name for the nursery included in the request.

During the process:

1. Filled-out application form to change the nursery’s trade name.

2. Commercial registration for the nursery with the new trade name.

3. Certified lease agreement for the nursery building with the new trade name (if the lease is in the nursery’s name).

4. Kahramaa (electricity and water) bill with the new trade name for the nursery (if the account is under the nursery’s name).

5. Traffic license with the new trade name for the nursery.

6. Civil defense certificate with the new trade name for the nursery.

7. Approval document for the installation of surveillance cameras under the new trade name from the Security Systems Administration at the Ministry of Interior.

8. Trade license with the new trade name for the nursery.

9. Establishment registration card with the new trade name for the nursery.

10. License for the first aid unit with the new trade name for the nursery.

11. Commitment to comply with the instructions issued by the Nursery Licensing Department at the Ministry of Education and Higher Education under the new trade name for the nursery.

12. Ministerial decision No. (6) of 2005 (Work Organization Regulation) and decision No. (7) of 2005 (Penalties Regulation) for the nursery, certified by the Labor Relations Department at the Ministry of Labor.

13. Original bank guarantee letter worth 100,000 QAR with the new trade name for the nursery.

14. Nursery regulations with the new trade name for the nursery.

1. The Nursery Licensing Department must approve the new trade name for the nursery before the license holder initiates the name change procedures.

2. The applicant must submit the trade name change request at least six months before the nursery license expires.
