1. Academic Sequence Statement Request Form (if the application is submitted in person).

2. A copy of the Qatari ID of the student or guardian (if the application is submitted in person).

3. A copy of the passport for non-Qataris of the student or guardian (if the application is submitted in person).

1. The student must be actively enrolled in a school in Qatar.

2. The Qatari ID must be valid.

3. The passport must be valid.

4. If the statement was issued before the academic year (2010–2011), a manual application must be submitted to the School Certificate Equivalency and Authentication Department at the Ministry’s headquarters (for government schools or private schools following the government curriculum).

5. If the statement was issued before the academic year (2020–2021), the statement must first be obtained from the school and then authenticated at the Ministry’s headquarters (for private schools not following the government curriculum).
