2. Guardian consent form (for the replacement student).

3. Required participation documents for the replacement student (if applicable).

1. The replacement student must meet the required conditions for the activity.

2. Reasons for the cancellation of the previous student's participation must be provided.

3. The request must be made sufficiently before the activity date (depending on the activity).
