3. Recent no objection letter from the employer for adding/changing the scope of practice.

4. Copy of additional academic certificates related to the new scope (if available).

5. Copy of additional experience certificates (with issuance date) related to the new scope (if available).

6. Copy of verification reports for any additional documents.

1. The practitioner must be tied to a specific workplace and is not authorized to practice elsewhere, except for Qatari nationals.

2. The practitioner's work must be limited to the profession/specialty stated in the license.

3. The passport must be valid.

4. The Qatari ID card must be valid.
