5.Copy of a food safety certificate (at least HACCP) (for factories).

6.Risk analysis (for factories).

7.Process flow diagram (for factories).

1.The applicant must complete operational information

2.The corrective action must comply with the requirements outlined in the Food Safety Practices Guidelines for Food Services available on the Ministry of Public Health’s Food Safety Department page.
