First: Qatari deceased individuals within the country:

1. Qatari ID card of the deceased.

2. Qatari ID card of the person reporting the death.

3. Death notification issued by the hospital (or a body release permit issued by the Public Prosecution if the death occurred outside the hospital).

Second: Qatari deceased individuals outside the country:

1. Qatari ID card of the deceased.

2. Death certificate issued by the country of death.

3. Qatari ID card of the person reporting the death.

Third: Non-Qatari deceased individuals within the country:

1. Passport of the deceased.

2. Qatari ID card of the deceased.

3. Qatari ID card or passport of the person reporting the death.

4. Death notification issued by the hospital (or a body release permit issued by the Public Prosecution if the death occurred outside the hospital).

5. Company representative card (if no first-degree relative is available).

The applicant must be a first-degree relative of the deceased or an authorized representative.
