First: Newborn Qatari Nationals Born Inside Qatar:
Father Qatari + Mother Qatari:
1.Qatari ID card of the father (copy).
2.Qatari ID card of the mother (copy).
3.Copy of the marriage contract.
4.Vaccination card with the health number (copy).
5.Notification of live birth issued by the hospital where the birth took place.
Father Qatari + Mother Non-Qatari:
1.Qatari ID card of the father (copy).
2.Qatari ID card of the mother (copy).
3.Passport of the mother (copy), including residence or entry visa (for non-GCC mothers).
4.Copy of the marriage contract.
5.Marriage approval from the Ministry of Interior (for non-GCC mothers).
6.Vaccination card with the health number (copy).
7.Notification of live birth issued by the hospital where the birth took place.
Second: Newborn Qatari Nationals Born Outside Qatar:
1.Qatari ID card of the father (copy).
2.Qatari ID card of the mother (copy).
3.Passport of the mother (copy) with entry visa (for non-GCC mothers).
4.Copy of the marriage contract.
5.Marriage approval from the Ministry of Interior (for non-GCC mothers).
6.Birth certificate (attested) from the country of birth.
7.Exit-Entry Permit.
Third: Newborn Non-Qataris Born Inside Qatar:
1.Passport of the father (copy) or entry visa for non-residents.
2.Passport of the mother (copy) or entry visa for non-residents.
3.Vaccination card with the health number (copy).
4.Notification of live birth issued by the hospital where the birth took place.
5.Copy of the marriage contract.
6.Marriage approval from the Ministry of Interior for Qatari wives.
