1.Qatari ID card of the deceased.

2.Death certificate issued by the country of death.

3.Qatari ID card of the notifier of the death.

Third: Non-Qataris Deceased Inside Qatar:

1.Passport of the deceased.

2.Qatari ID card of the deceased (if available).

3.Qatari ID card or passport of the notifier.

4.Notification of death issued by the hospital (or body release permit issued by the Public Prosecution if death occurred outside the hospital).

5.Work representative card (in the absence of a first-degree relative).

The applicant must be a first-degree relative of the deceased or an authorized representative.
