This service aims to issue a license for a pharmaceutical establishment after completing three main phases: initial approval, preliminary inspection, and final inspection to ensure compliance with regulations.

Incomplete Digital

First Phase:

1. Completed registration form for an existing building for pharmacies and drug warehouses.

2. Copy of the owner's Qatari ID card.

Second Phase:

1. Copy of the initial approval certificate for the pharmaceutical establishment.

2. Copy of the Good Conduct Certificate.

3. Copy of the lease contract for the establishment.

4. Site plan showing location and area.

5. Copy of the healthcare facility license (if requesting an internal pharmacy license).

Third Phase:

1. Copy of the preliminary inspection certificate for the pharmaceutical establishment.

2. Copy of the professional license for the responsible pharmacist.

3. Copy of the external signage for the establishment.

4. Copy of the commercial registration and trade license.

5. Copy of the Civil Defense certificate.

6. Approval from the Security Systems Department for surveillance cameras and devices (if a drug warehouse or internal pharmacy).

7. List of medical staff names.

8. Copy of the medical waste disposal contract.

9. Patient Rights Charter form.

10. Copy of the Administrative Job Blacklist/Prohibition form.

1. The applicant must be a Qatari national.

2. The applicant must be at least 21 years old.

3. The establishment must comply with the regulations and conditions outlined in Law No. 3 of 1983 regarding the practice of pharmacy and agents and brokers of pharmaceutical companies and factories.
