This service aims to issue surgical privileges to a licensed general physician, enabling them to gain approval to perform advanced surgical procedures. These privileges serve as a monitoring tool used by the Health Professions Department to ensure the competence and safety of healthcare practitioners.

1. Surgical privileges application form or a surgical privileges letter (if no specific form is available for the requested privilege).

2. Copy of the medical license.

3. No objection letter signed and stamped by the medical director of the medical facility where the physician is licensed.

4. 'Request for Approval to Conduct a Surgical Privilege' form, signed and stamped by the medical director of the medical facility where the surgical privileges will be performed.

5. Personal commitment letter.

6. Copy of academic qualifications.

7. Copy of CV.

8. Recommendation letter (if available).

9. Copy of the medical facility’s license indicating the services provided by the facility.

10. Letter from the medical facility where the surgical privileges will be conducted.

11. Surgery log of the past 3 years for the requested privileges (excluding dermatology).

12. Training courses in the requested privileges for surgical procedures (if available).

1. The medical license must be valid.

2. The medical facility's license must be valid.
