Reinstatement and Activation of Healthcare Practitioner License

This service aims to reinstate a healthcare practitioner's registration in the healthcare practitioners' registry in Qatar and change their license status from 'Inactive' to 'Active.'

Digital

1. Copy of the Qatari ID card.

2. Employment offer letter.

If the healthcare practitioner left Qatar and continued practicing abroad:

1. Copy of the criminal record certificate.

2. Copy of the medical report for licensing purposes.

3. Copies of experience certificates during the removal period, verified by DataFlow.

4. Certificate of Professional Good Standing (original or officially issued by the competent authority in the country of practice).

If the practitioner left Qatar but did not practice abroad:

1. Copy of the experience certificate from the last place of work in Qatar.

2. An explanatory letter.

If the practitioner remained in Qatar and applied for reinstatement within 2 years of removal:

1. Experience certificate from the last place of work in Qatar.

2. An explanatory letter.

1. The practitioner must be tied to a specific workplace and is not authorized to practice elsewhere, except for Qatari nationals.

2. The practitioner's work must be limited to the profession/specialty stated in the license.

3. The Qatari residency must be valid.

4. No more than 5 years should have passed since the practitioner's license was removed from the registry.
