Request to Reschedule an Appointment for Overseas Treatment

This service aims to modify overseas treatment appointments for patients who have already received a prior appointment, funded by the state.

Digital

1. Copies of passports (for the patient).

2. Copies of passports (for the companions).

3. IBAN certificate.

4. Medical report.

5. Disability certificate (for people with special needs).

6. Certificate of unmarried status (if applicable).

7. No objection letter (for military companions).

1. The patient must have a medical record and approval number.

Companion Requirements:

1. The companion’s age must be between 18 and 65 years.

2. The companion must not have health issues that prevent them from caring for the patient.

3. If the companion has chronic illnesses, they must bring their medications for at least two months.
